General Information
Office Hours
We are open Monday to Friday 7am-7pm.
The office is closed during statutory holidays.
Parking
Parking is free. The MNP Place has a large surface parking lot and is equipped with charging units for electric vehicles.
Location
We are located inside MNP Place on the ground floor at the end of the hall. MNP Place can be found beside the Group Health building off 31st Ave. Our address is #102 – 15303 31st Ave. Surrey, BC V3Z 6X2
What to Wear
Please wear or bring suitable exercise clothing for each visit.
Service Rates & Billing
We accept Interac, VISA, Mastercard, AMEX, Union Pay and Cash
Physiotherapy Service rates
Physiotherapy services at Diane Lee & Associates are provided by registered physiotherapists and our rates are based on the Physiotherapy Association of BC fee guidelines. Effective January 1, 2025 our rates for physiotherapy will increase slightly for the first time in two years to:
1 hour $195
45 minutes $146
30 minutes $97.50
MSP Supplementary Benefits
Should you qualify for MSP Supplementary Benefits, we can partially bill physiotherapy treatments directly to your MSP. MSP contributes $23 to each session for a maximum of 10 visits per calendar year. These treatments include other health providers that may be helping with your care.
WorkSafe BC
Diane Lee & Associates does not have a contract with WorkSafe BC and cannot offer physiotherapy services related to WorkSafe BC injuries and/or claims.
ICBC
Diane Lee & Associates does not have a vendor number with ICBC and we do not offer direct billing for ICBC clients. Patients are responsible to arrange for coverage and reimbursement of all physiotherapy services, treatment plans and reporting expenses.
Individual Extended Health Plans
Direct billing is only available for physiotherapy services to patients with coverage through RCMP and Veterans Affairs Canada as well as patients on a UBC Joint Replacement program. Please contact the front desk for more information: 604-538-8338
We do not direct bill to other third-party insurers. Physiotherapy coverage varies from plan to plan so it is important that you confirm your coverage prior to booking as patients are responsible to arrange for reimbursement
Registered Massage Therapy Service rates
Massage therapy services are provided by registered massage therapists and the rates are based on the Registered Massage Therapists Association of BC fee guidelines.

WorkSafe BC, ICBC, MSP & Individual Extended Health Plans
Our Massage therapy program is not currently accepting patients with ICBC or WorkSafe BC related injuries and/or claims. Additionally, we do not offer direct billing to individual extended health plans or MSP Supplementary benefits.
West Coast Pedorthics Service and Product Fees
Customs foot orthotics, gait analysis and custom knee & ankle bracing is offered at our clinic by a certified pedorthist from West Coast Pedorthics.
Both off-the-shelf and custom products vary in price. For pricing information, you can contact our certified pedorthist anytime by email:
West Coast Pedorthics offers direct billing to the following third-party payers: RCMP, Canadian Armed Forces, Veterans Affairs, ICBC, WCB, First Nations Health Authority or for anyone who is on Disability and Ministry Assistance with the province.
Yoga Therapy Service rates
Yoga therapy is provided by a certified yoga therapist. Yoga therapy is not currently billable to third-party payers.

First Visit

Returning Patients
For patients who have seen us in the past and have a new concern; we invite you to contact the office @ 604-538-8338.
Our front desk team can ensure you are scheduled with a clinician suitable for your current needs and update you on any changes in the clinic since your last visit with us.
Booking Online
Some clinicians allow online booking through our management website. Please call the clinic for your first appointment so we can ensure you are booked with the best person for your condition.
Once your account is set up you can book appointments with these clinicians and cancel with all clinicians online (up to 24 hours before your appointment).
Booking By Phone
For clinicians not offering online booking, all appointments are scheduled by phone, please call us at 604 538 8338.
Clinic Policies
Sick Policy
We continue to do our part to keep everyone safe and ask that you do the same. We ask our patients to be 3-days symptom free before coming into the clinic for your appointment.
In the case that you are not able to attend due to illness, our clinic offers telehealth appointments to do your session virtually instead.
Late arrivals
We do our best to run on time and we respect that your time is as valuable as ours. If you arrive late, your time with the therapist will be shortened to avoid delaying those scheduled after you.
No show and short notice cancellation policy
If you can no longer attend your scheduled appointment, we do expect a call or email at least 24-hours ahead of time. No notice for cancellations within 24-hours are subject to a 100% charge of the session fee.
Reminder notifications are sent through email, (48 hours) and text message (24 hours) prior to your visit.

JANE is a valuable tool that allows you to check your upcoming appointments, cancel any unsuitable appointments online (24 hours prior), and update your registration details at any time.

Review Exercises
In addition, your therapist can upload video and pictures of your home exercise/therapy program to your account for home review.
